Please e-mail Annette Brown at NYU.  She will send you a packet with 3 brushes to obtain a cheek swab sample from your dog.  You will also receive detailed instructions for obtaining the sample and mailing instructions.

When mailing back the samples please print and enclose the application (see steps below).

Application: 

1. Print and complete the test application form with all the required information.  If more than one dog is being tested fill out a separate application for each dog.  Be sure that the printing is clear and legible.  Attach a copy of the dog's AKC/CKC registration.
2. Enclose your application form and check made out to the NYU Department of Neurology (Phone: 212-263-2943). $120.00 for each dog, $98.00 per dog if six or more samples are sent together.
3. Send package PRIORITY MAIL to:

*Your results will be mailed directly to you from NYU in approximately 2 - 3 weeks.
